Founded in December 2019, top and dedicated Nurse Practitioners and Doctors working collaboratively with one goal to bridge the gap of care by bringing Compassionate Care to those in the comfort on their homes. Access Family Health NP PLLC also works close with all major local Nursing Homes, Assisted Livings and Group Homes to render “Patient-Centered Care” by focusing on those living at home and in community-based settings and who are dealing with preventable and or chronic diseases by providing optimal care, raising awareness and disease prevention including patient and staff education. We also partner with some of the major laboratories and in-home XRAY in New York State such as QUEST, EMPIRE, ACUTIS and NY Home XRAY to bring diagnostic care to our patients.
Access Family Health NP PLLC believes in continuing education! We're continually working hard to improve health and care within the communities throughout the 5 boroughs including Long Island. Our dedicated nurse practitioners and doctors are always working hard to keep patients involved in the decision about their care!
We also offer staff in-service on a monthly basis to all major local Nursing Homes, Assisted Livings and Group Homes contracted with Access Family Health PN PLLC to provide continuous education on preventive and chronic diseases.
We offer medical PCP and sick visits, perform COVID 19 swab, rapid PCR-result within minutes and Antibody testing, IGG, for patients at home, Nursing Homes, Group Homes as well as Assisted Livings to test staff and patients/clients.
